Background: Poor community health awareness leads to delayed risk identification and underuse of early screening. Purpose: To enhance health awareness through a structured health-check service integrated with concise practical education. Methods: Partner-based activities with 18 participants included outreach, brief visual counseling, a hands-on workshop on self-monitoring (blood pressure, BMI, random blood glucose), and lifestyle counseling; evaluation comprised pre- and post-knowledge tests, skill observation checklists, satisfaction surveys, and follow-up plans. Results: Health screening of 18 older adults showed 44% with hypertension (150/95 mmHg), 28% with hyperglycemia (215 mg/dL), 33% with hyperuricemia (7.8 mg/dL), and 39% with hypercholesterolemia (230 mg/dL), indicating a high prevalence of abnormal conditions among the elderly. Conclusion: Integrating health checks with brief education and hands-on counseling is a feasible community-level strategy to raise health awareness.
